When the enrolment form is signed and returned, it becomes a contract for professional services and failure to attend the EDA summer school, in part or in full, in no way absolves the applicant from his or her total financial obligations. Payment for the EDA summer school must be paid in full to confirm place. You can pay this electronically using your child’s name as reference to: 80-22-60, 15573266. Please note that EDA is only responsible for your child during the time of EDA Summer School. Parents should always wait for the class to begin before leaving the studio and always be there on time to collect their child. I hereby confirm that I understand and agree to the Terms and Conditions of Enrolment as set out above.

By signing this agreement, you are giving us permission to pass on your contact details to keep in line with the Scottish Governments Covid19 Test and Protect Scheme. 

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren)and I may be exposed to or infected by COVID-19 by attending EDA and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 EDA may result from the actions, omissions, or negligence of myself and others, including, but not limited to, EDA participants and their families.

On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge or lay blame to EDA, its employees, agents, and representatives should I or my child(ren) contract COVID-19. I agree to be forthright and true with EDA should I or my child(ren) show symptoms of illness, test positive for COVID-19, come in contact with people or persons showing symptoms of illness, or come in contact with people or persons who have tested positive for COVID-19. Should I choose not to wear a mask, I will continue to follow all safety precautions, including but not limited to, frequent hand washing, maintaining a safe social distance of at least 2 meters.

From time to time school photographs may be used to update Edinburgh Dance Academy publicity material and our social media platforms. Photographs will only be published once permission has been sought using this form and will only be used for legitimate EDA purposes.

I do/do not give permission for my child to be photographed/filmed whilst participating in summer school.

I do/ do not give permission for photographs/videos of my child to be used on the EDA Facebook page, website and other publicity materials.
